ENhancing COMmunity health through Patient navigation, Advocacy, and Social Support (ENCOMPASS) Study
Researchers from the University of Calgary and the University of Alberta’s School of Public Health have partnered with Edmonton Oliver PCN and the Westview PCN to develop, implement, and evaluate a community health navigator program for patients with multiple chronic health conditions. The intervention provides patients with a CHN for 6 months. The CHN will help patients navigate the health system, facilitate communication with providers, improve patient understanding of their conditions and treatment plans, meanwhile cultivating a partnership with patients and empowering them to take ownership of their health care and self-management of their conditions.
